Count first, adjust second
A stocktake should establish a defensible physical count, not simply force a dispensing system's value to zero and rebuild it without an audit trail. Use the practice's approved stock-count procedure, protect any ongoing dispensing activity and preserve a snapshot of the system position before changes are made.
Reconcile more than shelf stock
For each medicine pack, separate four quantities: the system balance, physically counted stock, outstanding supplier deliveries and stock already committed to prescriptions or owings. A delivery that has not arrived is not on the shelf, but it can still affect how much needs to be ordered next.
Use a stable product identity
Group records at the level of medicine, formulation, strength, brand where relevant and exact pack. Similar supplier descriptions can hide different devices, volumes or pack sizes. A reliable dm+d-led crosswalk helps stop false duplicate merges and incorrect value comparisons.
Investigate differences rather than hiding them
- Receipts and returns: check whether deliveries, returns and credits were posted into the stock system.
- Owings and part supplies: check whether stock left the shelf without the usual final transaction.
- Pack conversions: identify lines counted in individual units but recorded in packs, or vice versa.
- Duplicates: look for separate lines for equivalent descriptions while preserving legitimate brand, form or device distinctions.
- Expiry and damage: segregate unusable stock and follow local disposal procedures rather than counting it as available to dispense.
A reconciliation worksheet that staff can action
A practical worksheet shows original product identifier and description, counted quantity, system quantity, variance, transaction evidence, proposed correction, reason and reviewer. Preserve before-and-after values for every change. Record whether a variance is understood or still awaiting investigation.
Protect high-risk stock
Controlled drugs require the applicable statutory records, security and independent checking arrangements; a general spreadsheet cannot substitute for them. Refrigerated stock also needs appropriate cold-chain monitoring, expiry checks and storage controls. Escalate discrepancies under the practice's clinical-governance process.
Turn the completed count into a sensible catch-up order
- Finish and verify the physical count in a controlled way.
- Reconcile outstanding transactions, deliveries and known owings.
- Approve traceable corrections to the stock system.
- Review regular versus irregular stock status based on actual dispensing history.
- Calculate net replenishment need against agreed min/max policy, after allowing for items already on order.
- Use verified exact-pack supplier comparisons to build baskets.
- Retain a dated review of the stocktake, exceptions and resulting purchases.
